The IPC GPP Training is a five-part educational series designed to provide clinicians with a practical foundation in the diagnosis, assessment, and evaluation of Generalized Pustular Psoriasis (GPP). Through a combination of expert-led educational videos and case-based exercises, participants will build confidence in recognizing and assessing GPP in clinical practice.

For those seeking a Certificate of Completion, an assessment follows each module. Participants must complete all five assessments and achieve a score of 80% or higher on each assessment to earn the certificate. The assessments are designed to reinforce key concepts and support mastery of the core competencies presented throughout the course.
This module introduces the IPC consensus definition and diagnostic criteria for generalized pustular psoriasis (GPP) and shows how the condition differs from chronic plaque psoriasis in presentation, genetics, and immunopathogenesis. It also reviews current and pipeline treatment options, including spesolimab trial data and off-label therapies.
This module walks through conditions that can mimic GPP, including bacterial, viral, and fungal infections, AGEP, Sneddon-Wilkinson disease, pemphigus, linear IgA bullous dermatosis, and DIRA. Clinical photos and histopathology highlight the features that distinguish each condition from GPP.
This module covers the GPPGA and GPPASI tools for scoring the cutaneous severity of GPP and the Japanese Dermatological Association criteria for evaluating systemic involvement. It also discusses incorporating patient-reported outcomes, such as fatigue, into a full severity assessment.

A patient doing well on an IL-17 inhibitor is diagnosed with cancer, and the oncologist wants the biologic stopped. It is a call most dermatologists have made without much to stand on: patients with cancer are excluded from psoriasis trials, so the entire evidence base is observational. A new IPC paper in JAAD Reviews takes a position anyway. Per the summary of product characteristics, active malignancy is not a contraindication to any biologic. IL-17 and IL-23p19 inhibitors are first-line, TNF inhibitors and deucravacitinib warrant caution, and cyclosporine is the one to avoid outright. The authors also warn that psoriasis severity is routinely underestimated once a patient is in oncology care: if they meet IPC’s criteria, they are a candidate for systemic therapy regardless of what else is on their problem list. “Too often, systemic psoriasis therapy is discontinued or never initiated in patients with cancer, even when the evidence supports safe and effective options,” says Paolo Gisondi, MD, IPC Councilor and lead author.
